If you are scheduled for surgery and choose to file a claim with your Insurance Provider the prices on the
website DO NOT apply.

The prices listed include the Facility Fee, the Surgeons Fee, and the Anesthesiologist Fee.

The following items are not included:

• Any diagnostic tests or studies including pathology
• Lab work
• CT, MRI, X-Rays, etc.
• Any expenses or fees resulting from complications are also not included

The pricing listed is not negotiable and is available only to those who pay the full amount in advance.
If you are part of a self-funded insurance plan or have a high deductible and DO NOT intend to file an
insurance claim you may qualify for this program. A list of procedures and prices are listed on our
website. The prices only include the Facility, Surgeon and Anesthesiologist Fees and DO NOT include any
outside labs, diagnostic tests, pathology, or office visits, etc.
